TidePointe, A Vi Community, on beautiful Hilton Head Island, SC is looking for a MDS Coordinator with 2-5 years experience.
This individual will be responsible for conducting and coordinating completion, collaborating with all disciplines, of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) assessment and all associated processes. Signs and attests to completion of the assessment within the timeframe mandated by federal regulations and company standards. Also responsible for the coordination of the resident care planning processes.

Current State Licensure as a Registered Nurse. Experienced MDS and Medicare Guidelines.

TidePointe, A Vi Community, on Hilton Head Island has an opening for a FT Registered Nurse. The RN is responsible for the delivery of nursing care to the residents on his/her assigned area during a given period of time. RN responsibilities include assessment, diagnosis, planning, implementation and evaluation. In addition, the RN is responsible for supervision of assigned staff members.

The ideal candidate for this position will be a graduate from accredited school of nursing. Bachelor’s Degree preferred. Experience working in long-term care or in geriatrics a plus.

The Licensed Practical Nurse is responsible for the delivery of care and services to the resident in his/her assigned area during a given period of time. Delivery of resident care is performed through the identification of resident needs and prescribed interventions.

Education and Experience:

• Education: Graduation from a School of Nursing for Licensed Practical/Vocational Nurses.
• Work Experience: Prior experience working with geriatric clients preferred.
• Licensure / Certification, where required: Current State Licensure as a licensed practical nurse or licensed vocational nurse, in state where practicing.

Cardiopulmonary resuscitation/Automatic External Defibrillator certification. First Aid certification, required
